"Dann kommt das Krankenhaus eben zu Ihnen." – Klinisches Schnittstellenmanagement und subjektiver Bedarf von Eltern onkologisch erkrankter sterbender Kinder in der häuslichen Palliativversorgung. Ergebnisse einer Evaluationsstudie.

dc.description.abstractTrotz erheblich gestiegener Heilungschancen in der pädiatrischen Onkologie ist jährlich für mehr als 500 Kinder und Jugendliche in der Bundesrepublik Deutschland eine Genesung von ihrer Krebserkrankung nicht möglich. Die betroffenen Kinder und Jugendlichen sterben. In der international anerkannten Leitlinie zur pädiatrischen Palliativversorgung – der ACT-Charta – wird festgelegt, dass die häusliche Umgebung der Familie das Zentrum der Versorgung sein sollte, wann immer das möglich ist. Im deutschen Gesundheitssystem ist die häusliche pädiatrisch-onkologische Palliativversorgung im Rahmen eines klinikgestützten „Hospital at Home“-Konzepts bisher rechtlich und finanziell nicht verankert. Das Ziel der in der vorliegenden Studie exemplarisch untersuchten universitären Klinik der Maximalversorgung ist es gleichwohl, eine Integration der Versorgung in der häuslichen Umgebung sicherzustellen. Darüber, wie vor diesem gesundheitssystemischen Hintergrund Eltern den Übergang nach Hause und die dort erfolgte professionelle Palliativversorgung durch die KlinikerInnen und einen kooperierenden ambulanten Pflegedienst in der letzten Lebensphase ihres Kindes erlebt haben und wie sie sich nach dessen Tod in der Nachsorge begleitet fühlten, ist bislang wenig bekannt. Hier setzt die Untersuchung an. Im Zentrum des Studiendesigns stehen elf leitfadengestützte problemzentrierte Interviews mit verwaisten Eltern, die inhaltsanalytisch ausgewertet wurden. Ergänzend wurden zehn halbstandardisierte ExpertInneninterviews mit MitarbeiterInnen des klinischen Teams und des externen Pflegedienstes durchgeführt und ebenfalls inhaltsanalytisch ausgewertet. Ziel der Studie war es, den subjektiven Bedarf der Eltern zu ermitteln, Erkenntnisse über deren subjektive Qualitäts- und Zufriedenheitskategorien zu sammeln und subjektive Kriterien der Inanspruchnahme bzw. Nicht-Inanspruchnahme von Unterstützungsangeboten zu erforschen. Evaluiert wurde der gesamte Prozess der Versorgung ab der Diagnosestellung bis zur Nachsorge der verwaisten Familie. Übergeordnete Fragen an die verwaisten Eltern waren: Welches Versorgungsangebot wurde aus ihrer Sicht an der Schnittstelle und im Verlauf der häuslichen Palliativversorgung bis in die Nachsorge zur Verfügung gestellt?, Wodurch und durch wen fühlten sie sich von Seiten der professionellen VersorgerInnen unterstützt? und schließlich: Wodurch und durch wen fühlten sie sich nicht ausreichend unterstützt oder zusätzlich belastet? Unter den zentralen Kategorien der Kommunikation, Koordination und Kooperation, unterschieden nach den einzelnen Berufsgruppen und differenziert nach den einzelnen Versorgungsabschnitte werden wichtige Erkenntnisse über die onkologische Pädiatrische Palliativversorgung zur Verfügung gestellt.de
dc.description.abstractDespite the significantly increased survival rates in pediatric oncology, in Germany every year more than 500 children and youths do not recover from their cancer and succumb to their illness. The internationally recognised guidelines on pediatric palliative care, known as the ACT charter, have determined that the family’s home environment should form the centre of palliative care whenever possible. To date in Germany the concept of the „hospital at home“ as a way of providing home-based palliative care in the context of pediatric oncology has been neither legally nor financially established. Nevertheless the key objective of the university hospital examined in this study has been to ensure continuity of care is provided, including the integration of home-based palliative care. So far little has been known about how parents perceive the transition to the home environment and the subsequent professional palliative care provided by hospital staff and the cooperating outpatient care provider during the final stage of their child’s life. Neither has it been known how parents perceived the bereavement care provided to them following the child’s death. These issues are the focus of this study. The structure of the study revolves around eleven standardised problem-centred interviews that were conducted with bereaved parents and evaluated through content analysis. In addition to these interviews with parents, ten partly standardised interviews were conducted with health professionals at the hospital and staff of the outpatient care provider. These interviews were also evaluated through content analysis. The aim of the study was to ascertain the perceived needs of the parents, to identify the subjective quality and satisfaction categories, and to establish what subjective criteria informed their decision for utilisation or non-utilisation of the different types of support offered to them. The evaluation encompasses the complete process of care, from the initial diagnosis to the bereavement care provided to the family following the child’s death. The key questions presented to the bereaved parents were: What support had they experienced both at the interface to and in the course of the home-based palliative care and the subsequent bereavement care? What type of support and whose support among the range of health professionals had they considered to be the most significant? And finally, what type of support and whose support among the range of health professionals had been lacking or had been experienced as an additional burden? Based on the three categories of communication, coordination and cooperation, and differentiating between different groups of health professionals and different stages of the overall care experience, the study provides important insights into the palliative care provided in the context of pediatric oncology.en
